Readmission Readmission

1. Fill out Application for Readmission Form, which is available at your Learning Center and Downloadable Forms.

2. Pay the application fee of P225. See payment options and procedure for local students.

3. Submit accomplished form and payment slip before the set deadline for the term. 

4. Submit the documents to the LC Coordinator. 

5. Dean's Office through the concerned Program Chair acts on application (including request for waiver of MRR) and advises student in courses to enroll and revised plan of study.

6. The Office of the University Registrar informs student of the result of application.
