To file your request for student ID, please complete the online request form at https://forms.gle/
Photo requirements:
(1) Colored photo with specific color background by degree program
> Orange background for PTC, PhD, Diploma, or Master’s program student
> Red background for undergraduate program (Associate programs, BAMS, or BES) student
(2) Must have been taken within the last six months
(3) Must clearly show facial features and taken in full-face view directly facing the camera
(4) Size: 2″ x 2″; should not exceed 500 KB each and must not be less than 100 KB. Minimum resolution of the file should be 350 pixels (Width) X 350 pixels (Height); the maximum is 1000 pixels (Width) X 1000 pixels (Height).
(5) Filename of photo: Use your student number

Applicants with a UPG of 2.800 or better, or waitlisted applicants who have not been offered a degree program may appeal through the DIWA portal starting 19 June 2025.![]()

Please be guided by the UPG cutoff score per UP campus. Note that some campuses or degree programs have additional requirements to appeal. Only degree programs and campuses for which you meet the appeals criteria and which have available slots will appear in the DIWA portal for you.![]()
